Insurance & Payments
Payment is due at the time of service in full. Member responsibility includes co-payments, deductibles, and coinsurance. Cash, check, American Express, Visa, and Mastercard payments are accepted.
We will submit insurance claims on your behalf, however, Mid Atlantic Retina will not become involved in the disputes between you and your insurance company regarding deductibles, co-insurance, covered charges, additional coverage’s, etc. Mid Atlantic Retina will supply requested information as necessary for further processing. Patients are responsible for the payment of their account.
We will submit all insurance claims for you, in which we are a participating provider, as long as you provide all the required information. Required information includes:
- A copy of your insurance card
- Photo ID
- An address and phone number to submit claims and verify coverage
Mid Atlantic Retina will submit claims to all insurance carriers, in which we are a participating provider. We request that patients bring their health insurance cards with them to appointments.
If a referral is required by your insurance carrier, it must be provided at time of service. Referrals must be obtained from your Primary Care Physician.
Any balance, after payments at the time of service and from processing of insurance claims, will be billed to the patient and must be paid within 20 days.
